Please fully complete the below forms.  

Signatures or initials from each adult in the household are required in the sections highlighted in yellow.  Completed forms can be returned to the Housing Help office during regular business hours or placed in the blue drop box on the porch – please be sure to drop it into the locked portion of the box. Applications may also be submitted at: info@h2lenawee.org

Please be advised that electronic signatures can not be accepted at this time.

All documentation listed below is required to complete a full and accurate assessment.

1. Identification – 1 form of ID for any adult – 1 form for any children: (only the following accepted)
   • Valid State of Michigan Drivers License or State ID with Lenawee Co. address shown
   • Social Security Card
   • Birth Certificate
2. Proof of ALL CURRENT INCOME (dated within past 30 days) for every adult applying, such as:
   • Payroll check stub
   • DHHS letter of eligibility for assistance (cash assistance or state disability assistance)
   • Social Security Income or Social Security Disability Income
   • Child Support
   • Proof of unemployment payment
   • Any other income verification that may apply (pension, retirement, etc.)
3. Proof of Lenawee County Residency (any of the following):
   • ID
   • Utility Bill
   • Lease agreement
4. Housing documents needed:
   • If you are currently residing at a shelter, please bring verification from that shelter
   • Eviction paperwork, if applicable. If court ordered, apply at DHHS too.
   • If you are staying with friends and family please provide a letter from them stating how long you can stay
5. Veterans:
   • Verification of Veteran status (DD-214)

After assessment, you will be contacted by phone for an appointment or by mail if ineligible for current funding.
